One of the questions is whether you have ever been involved in any sexual activity. Although the heading of the performa states that it is for both students and teachers, it is a question for innocent students who are ten or eleven years old. What will be the effect of such questions on their innocent minds? The said performa asks a lot of questions about medical health of students. There are questions about the parents’ surgical history, medical history, whether the child is underweight or overweight.
