The report underscores the value of comprehensive imaging in identifying serious underlying conditions beyond the initial clinical suspicion.

Published in the peer-reviewed journal Cureus, the report describes a patient who underwent echocardiographic imaging after presenting with symptoms suggestive of acute coronary syndrome. While assessing the heart, clinicians identified an abnormal mass extending into the right atrium, prompting additional diagnostic investigations. Further imaging confirmed advanced hepatocellular carcinoma with tumor extension through the inferior vena cava (IVC) into the heart.
